Job summary

Shook Home Health Care Center is seeking an LPN to provide compassionate patient care in a home health setting. The role entails conducting assessments, wound care, and medication-related duties while leading CNAs and maintaining high standards of care.

Candidates must hold an LPN license and demonstrate empathy, reliability, and strong teamwork to support residents and the care team.

Qualifications

• LPN licensure is required.
• Applicant should demonstrate empathy, patience and a caring approach.
• Ability to lead CNAs and CMTs while following facility protocols.

Responsibilities

• Conduct assessments, follow protocol, report medical or behavioral changes, and document findings
• Perform admission duties: complete assessment, MAR, TAR, POS, review chart set-up, physician orders, and order medications
• Manage discharges: complete documentation, notify physician and guardian, and arrange transportation
• Provide wound care: follow protocols, irrigate, pack, and remove dressings
• Lead CNAs and CMTs, ensuring compliance with facility protocols and policies
• Respond effectively to emergency situations
