Job summary

Human Touch Home Health seeks compassionate LPNs for PRN home health opportunities across Fredericksburg and surrounding communities. Provide skilled nursing visits in patients' homes, perform assessments, administer medications, and educate patients and families with supportive leadership.

Choose areas that work for you, build your caseload around availability, and benefit from mileage reimbursement and consistent patient opportunities.

Qualifications

• Active Virginia LPN license required.
• Graduate of an accredited nursing program.
• Current CPR/BLS certification.
• Valid driver's license and reliable transportation.
• Home health experience preferred but not required.
• WellSky experience is a plus.

Responsibilities

• Provide skilled nursing visits in patients' homes.
• Perform focused nursing assessments.
• Administer medications and treatments as ordered.
• Provide wound care and monitor healing.
• Monitor vital signs and changes in patient condition.
• Reinforce medication and disease-management education.
• Educate patients and caregivers.
• Communicate changes or concerns to the supervising RN and clinical team.
• Coordinate care with other members of the interdisciplinary team.
• Complete accurate and timely documentation using WellSky.

Human Touch Home Health is seeking compassionate and dependable Licensed Practical Nurses (LPNs) for PRN home health opportunities throughout Fredericksburg and surrounding communities.

Patient opportunities may include Fredericksburg, Stafford, Spotsylvania, King George, Caroline County, Orange, Locust Grove, Bowling Green, Colonial Beach, and nearby areas.

You do not have to cover the entire territory. Choose the areas that work best for you and build your caseload around your availability.
