Job summary

The Lakes Home Care is seeking a compassionate Licensed Practical Nurse (LPN) to work with pediatric and/or adult patients in their own homes in the Fort Myers area. Responsibilities include using the nursing process to assess, plan, implement, and evaluate patient care; providing skilled nursing services per prescribed orders; and monitoring physiologic and psychosocial changes.

Qualifications include an active LPN/LVN license in the state, a current TB test or chest X‑ray, a valid BLS card,

• TB test or Chest X-Ray evidence.
• BLS certification current.
• One year nursing experience preferred.

The Lakes Home Care is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to work with Pediatric and/or adult patients in their own homes.

Responsibilities:

• Utilize the nursing process to assess, plan, implement, and evaluate patient care.
• Provide skilled nursing care/services in accordance with prescribed orders on each assignment.
• Assess signs and symptoms indicating physiologic and psychosocial changes in the patient’s condition.
• Collect, analyze, and interpret data and information from healthcare members and documents actual and/or potential nursing diagnoses.

Qualifications:

• Currently licensed as an LPN/LVN in the state in which the LPN will practice.
• Current TB or Chest X-Ray.
• Current BLS card.
• One year prior professional nursing experience preferred.
