Job summary

Chatham Hills Subacute Care Center seeks a Licensed Practical Nurse (LPN) for day shift. You will provide comprehensive nursing care to residents in the SNF and long-term care units, including med administration, vital signs, and family education. You will collaborate with physicians, NPs, and a care team while supervising CNAs to ensure quality outcomes.

• Current Licensed Practical Nurse (LPN) license.
• Experience in long-term care or skilled nursing preferred.
• Ability to collaborate with physicians, NPs and families.

Responsibilities

• Administer medications, including IVs, and monitor responses.
• Take and document vital signs and clinical observations.
• Provide direct resident care and therapies with outcome monitoring.
• Educate residents and families about care plans.
• Communicate with physicians, NPs, families and other health team members.
• Supervise CNAs and support staff.
