Job summary

Chatham Hills Subacute Care Center is seeking an experienced Licensed Practical Nurse (LPN) for the day shift. The role involves caring for residents in our Skilled Unit and Long Term Care Unit, administering medications, monitoring vital signs, and coordinating care with physicians and the broader health team.

Candidates must hold a current LPN license. Experience supervising CNAs and delivering resident and family education is a plus. We offer benefits and a supportive, fresh-care approach.
