Job summary

Haven Homes, a Cassia community, is hiring Licensed Practical Nurses (LPNs) to join our stable, caring team in Maple Plain, MN. You will provide compassionate, high‑quality care, administer medications, and collaborate with staff to ensure the best outcomes for residents.

This full‑time role offers a varied schedule, including every other weekend, with shifts 6:00 AM–2:30 PM or 2:00 PM–10:30 PM. Wage range is $28.00–$34.66 per hour, based on experience, with benefits through Cassia.

Qualifications

• Must have MN LPN License and be in good standing.
• Prior long-term care experience preferred.
• Strong communication and organizational skills required.
• Self-starter with a desire to be part of a team.

Responsibilities

• Displays courteous attitude and respect for residents, families, and staff.
• Administers medications and/or treatments per physician orders and monitors effectiveness.
• Manages medications, supplies, and equipment per policies and procedures.
• Documents care using computerized medication administration and charting software.
• Notifies Clinical Manager, attending physicians, and family of significant changes in resident condition and follows up as needed.
• Performs additional duties as required.

Licensed Practical Nurse Responsibilities

• Manages medications, supplies, and equipment including ordering, receiving, storing, and disposing of all items in accordance with policies and procedures.
• Documents care using computerized medication administration and charting software.
• Notifies Clinical Manager, attending physicians, and family members of significant changes in a resident's condition, and takes follow up action as necessary.
• Performs additional duties as required.

Licensed Practical Nurse Qualifications

• Must have MN LPNLicense and be in good standing.
• Prior LTC experience preferred.
• Must have strong communication and organizational skills.
• Self-starter with a desire to be a part of a team.
