Qualifications

• MN LPN license in good standing (Minnesota).
• Prior long-term care (LTC) experience preferred.
• Strong communication and organizational skills.
• Self-starter with a team-oriented mindset.

Responsibilities

• Displays a courteous attitude and respect for all residents, families, and staff.
• Administers medications and/or treatments per physician's orders and monitors effectiveness.
• Manages medications, supplies, and equipment per policies and procedures.
• Documents care using computerized medication administration and charting software.
• Notifies Clinical Manager, physicians, and family of significant changes in a resident's condition and follows up.
• Performs additional duties as required.
