Job summary

Forest Hill Center for Rehabilitation and Healing is hiring a Licensed Practical Nurse (LPN) to join a compassionate, patient‑centered team. Earn $32–$39 per hour with flexible schedules, full‑time, part‑time, and per diem options, and a supportive leadership group dedicated to your professional growth.

As an LPN, you will administer medications, coordinate admissions and transfers, collaborate with physicians and families, and contribute to excellent resident outcomes in a long‑term care

Qualifications

• Active LPN license in New Jersey.
• CPR certification required.
• Strong communication and teamwork skills.

Responsibilities

• Administer medications and treatments safely and accurately.
• Coordinate resident admissions, discharges, and transfers.
• Communicate with physicians, pharmacies, residents, and families.
• Monitor residents' conditions and report changes promptly.
• Collaborate with the interdisciplinary care team to provide outcomes.
• Implement and evaluate individualized care plans.
• Maintain accurate clinical documentation.
• Follow infection control, safety, and emergency procedures.
• Supervise nursing assistants and support staff as appropriate.
• Ensure resident dignity, privacy, and rights are protected.
• Maintain knowledge of nursing equipment and proper usage.
• Provide compassionate nursing care in a long-term care setting.
