Job summary

The Jewish Home for Rehabilitation and Nursing in Freehold, NJ is seeking a licensed practical nurse (LPN) to join our compassionate team. This facility offers FT, PT and Per Diem positions with shifts at 7 AM-3 PM, 3 PM-11 PM, and 11 PM-7 AM.

The LPN provides patient-centered care under the direction of the nurse manager and interdisciplinary team. Responsibilities include medication administration, following care plans, monitoring residents, documentation, and maintaining a safe environment

Qualifications

• Current, unencumbered active LPN license in New Jersey.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with standards, plan of care, and physician's plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments per plan of care and facility policies.
• Monitor residents’ conditions, report changes promptly to supervising staff, and respond to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders and regulatory standards.
• Support a safe, respectful, therapeutic environment for residents and families.

Shifts: 7 AM-3 PM | 3 PM-11 PM | 11 PM-7 AM
PRN: Any 8-Hr Shift (1 Weekend/Month)
$30-$38/hr
