Job summary

The Jewish Home for Rehabilitation and Nursing is seeking a Licensed Practical Nurse (LPN) to join our compassionate care team. We offer FT, PT, and Per Diem positions with shifts 7 AM–3 PM, 3 PM–11 PM, and 11 PM–7 AM, plus PRN 8-hr shifts on weekends.

Daily pay available and more! As an LPN, you will provide patient care under the plan of care, document accurately, administer medications, and collaborate with an interdisciplinary team to support residents' wellbeing.

Qualifications

• Active NJ LPN license in good standing.
• BLS/CPR certification required.
• Ability to manage multiple patients in a fast-paced setting.
• Strong clinical judgment and communication skills.

Responsibilities

• Provide patient care in accordance with the plan of care and physician orders.
• Follow and implement individualized care plans by Nurse Manager and team.
• Administer medications and prescribed treatments per care plan.
• Monitor residents, report changes promptly to supervising nurse or physician.
• Maintain accurate documentation per orders, plans, and standards.
• Support a safe, respectful, and therapeutic environment for residents and families.

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.

• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of New Jersey.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.
