The Jewish Home for Rehabilitation and Nursing is seeking a Licensed Practical Nurse (LPN) to join our team in a full-time, part-time, or per diem capacity. Shifts run 7 AM-3 PM, 3 PM-11 PM, and 11 PM-7 AM; PRN options and daily pay are available.

The role emphasizes compassionate, resident-centered care within a 150-bed sub-acute and long-term care setting. Responsibilities include medication administration, following care plans, and documenting care while maintaining a safe, respectful

Qualifications

• Current, unencumbered active LPN license in New Jersey.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care in accordance with standards and the plan of care.
• Follow and implement individualized care plans developed by the Nurse Manager.
• Perform medication administration and prescribed treatments.
• Monitor residents’ conditions and report changes promptly.
• Maintain accurate, timely documentation.
• Support a safe, respectful, and therapeutic environment for residents.

Shifts: 7 AM-3 PM | 3 PM-11 PM | 11 PM-7 AM
PRN: Any 8-Hr Shift (1 Weekend/Month)
$30-$38/hr

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):

• Current, unencumbered active LPN license in the state of New Jersey.
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
