Job summary

Laurel Brook Rehabilitation and Healthcare Center is seeking a licensed practical nurse (LPN) to join a compassionate team. You will provide patient care in accordance with the plan of care and physician orders, administer medications, and monitor residents’ conditions in a fast-paced setting.

Qualifications

• Current, unencumbered active LPN license in New Jersey (state).
• Valid BLS Certification (CPR) required.
• Ability to care for and manage multiple patients in a fast-paced healthcare environment.
• Strong clinical judgment, communication skills, and attention to detail.

Responsibilities

• Provide patient care per standards, plan of care, and physician's order.
• Follow individualized care plans developed by Nurse Manager and team.
• Administer medications and treatments per plan and policies.
• Monitor residents' conditions and report changes promptly to supervising staff.
• Maintain accurate documentation per orders, care plans, and regulatory standards.
• Support a safe, respectful, therapeutic environment for residents and families.

Weekend Baylor 7 AM- 11:30 PM, 3 PM - 7:30 AM

PRN: 7 AM- 3 PM, 3 PM- 11 PM, 11 PM - 7 AM

$3 PIB

$30 - $35 per hour (All inclusive)

Responsibilities for Licensed Practical Nurse (LPN):

• Provide patient care in accordance with accepted standards of nursing practice, the written plan of care, and the physician’s plan of treatment.
• Follow and implement individualized care plans developed by the Nurse Manager and interdisciplinary team.
• Perform medication administration and prescribed treatments in accordance with the plan of care (POC) and facility policies.
• Monitor residents’ conditions, report changes promptly to the supervising nurse or physician, and respond appropriately to clinical needs.
• Maintain accurate, timely documentation in accordance with physician orders, care plans, and regulatory standards.
• Support a safe, respectful, and therapeutic environment for residents and their families.

Qualifications for Licensed Practical Nurse (LPN):
