Job summary

Rising Ground, Inc. is seeking a Licensed Practical Nurse (LPN) for a full-time, on-site role in the Bronx, NY. Immediate start with a 7am–3pm shift in clinical and residential program settings.

The LPN will supervise day-to-day health services activities, deliver preventive care, and support youth in foster care and residential programs. Qualified candidates hold an active NY LPN license, have at least 2 years of nursing experience, and current BLS certification.

Qualifications

• Current LPN license in New York State, in good standing.
• Minimum of 2 years of nursing experience in a clinical, community-based, or related setting.
• Current Basic Life Support (BLS) certification.
• Ability to perform comprehensive nursing assessments and exercise sound clinical judgment.
• Strong critical thinking and problem-solving skills; able to interpret medical records.
• Experience triaging patient calls and coordinating follow-up care.
• Ability to guide and support staff while maintaining high standards of patient care.

Responsibilities

Role Overview

The LPN is responsible for supervising day-to-day health services activities. Health Services provide services and support for foster care, residential and ACS involved youth..

Key Responsibilities

• Provides general medical care and treatment to individual supported per program, on site.
• Performs physical examinations and preventative health measures within prescribed guidelines and instructions.
• Provide health evaluations, assessments, and screenings with follow-ups.
• Review all Pediatric guidelines for care, including immunizations and testing
• Records physical findings based on the patient’s condition.
• Participate in clinical team and other health professionals to prepare comprehensive patient care plan utilizing historical and current information
• Conducts and assists in medical examinations of youth after incidents.
• Provides health education to youth supported, including reproductive health education
• Administer Medication
• Train staff
• Assist with community provider visits as needed
• Other related duties as assigned by supervisor.

Required Qualifications

• Current Licensed Practical Nurse (LPN) license in New York State, in good standing.

Position Type/Expected Hours of Work
