Job summary

Rising Ground in The Bronx, NY is seeking a Licensed Practical Nurse (LPN) for a full-time day shift from 7am to 3pm. The LPN will oversee day-to-day health services in foster care, residential and ACS programs, providing medical care, examinations, immunizations, medication administration, and staff training.

Requirements include a NY LPN license, 2 years of nursing experience, BLS certification, and strong clinical judgment. In-person work is required with potential site travel.

Qualifications

• Current Licensed Practical Nurse (LPN) license in New York State in good standing.
• Minimum of 2 years of nursing experience in a clinical, community-based, or related healthcare setting.
• Current Basic Life Support (BLS) certification.

Responsibilities

Role Overview

The LPN is responsible for supervising day-to-day health services activities. Health Services provide services and support for foster care, residential and ACS involved youth..

Key Responsibilities

• Provides general medical care and treatment to individual supported per program, on site.
• Performs physical examinations and preventative health measures within prescribed guidelines and instructions.
• Provide health evaluations, assessments, and screenings with follow-ups.
• Review all Pediatric guidelines for care, including immunizations and testing
• Records physical findings based on the patient’s condition.
• Participate in clinical team and other health professionals to prepare comprehensive patient care plan utilizing historical and current information
• Conducts and assists in medical examinations of youth after incidents.
• Provides health education to youth supported, including reproductive health education
• Administer Medication
• Train staff
• Assist with community provider visits as needed
• Other related duties as assigned by supervisor.

Required Qualifications

• Current Licensed Practical Nurse (LPN) license in New York State, in good standing.
• Minimum of 2 years of nursing experience in a clinical, community-based, or related healthcare setting.
• Current Basic Life Support (BLS) certification required.
• Demonstrated ability to perform comprehensive nursing assessments and exercise sound clinical judgment in an autonomous work environment.
• Strong critical thinking and problem-solving skills, with the ability to evaluate and interpret medical records, laboratory reports, and other clinical documentation.
• Experience triaging patient calls, assessing urgency, and coordinating appropriate follow-up care.
• Ability to provide guidance and support staffas needed while maintaining high standards of patient care.

Position Type/Expected Hours of Work

Full-time position-hours are 7am – 3pm. Work is conducted in clinical settings within program facilities and residential programs. This is an in-person position only
