Job summary

RiseWell Community Services is seeking a dedicated Licensed Practical Nurse (LPN) to join our Brentwood residential program in Brentwood, NY. This full-time role provides health services including medication management within a multidisciplinary team for individuals living with psychiatric disabilities.

Qualified candidates must be licensed LPN in NY, have a valid NY driver license and at least 1 year of psychiatric experience; fingerprinting and background checks are required.

Qualifications

• Currently licensed as a Licensed Practical Nurse by the New York State Education Department.
• Minimum of 1 year of psychiatric experience in a treatment or rehabilitation setting.
• Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Responsibilities

• Provides health services including medication management as part of a multidisciplinary residential team for individuals with psychiatric disabilities in a transitional licensed residential program.

This position offers a unique opportunity to make a meaningful impact in the lives of individuals living with serious mental illness by providing community-based, recovery-oriented support.
