Job summary

The Hamlet Rehabilitation and Healthcare Center at Nesconset seeks an experienced Licensed Practical Nurse (LPN) to provide skilled nursing care in our SNF and long‑term care units.

You will administer medications (including IVs), monitor vital signs, and deliver treatments while educating residents and families and coordinating with physicians and NPs. Supervisory duties for CNAs are included.

Qualifications

• Current LPN license in New York.
• Experience in skilled nursing or long-term care preferred.
• Ability to administer medications, including IVs, and monitor resident outcomes.
