Job summary

Crowne Health Care is seeking a Licensed Practical Nurse (LPN) – Med Nurse for our long-term care and rehabilitation facility. Qualifications

• Current Licensed Practical Nurse (LPN) license or temporary PN license.
• Must pass background screening and drug screening.
• Long-term care experience preferred but not required.

Responsibilities

• Administer medications accurately according to physician orders and policies.
• Complete required documentation and resident assessments.
• Monitor residents and report changes to physicians and staff.
• Order and manage medications through the pharmacy.
• Supervise and provide direction to CNA staff.
• Provide wound care and treatments as ordered.
• Collect and process laboratory specimens.
• Complete assessments and documentation timely.

Qualifications

• Monitor residents for changes in condition and promptly report concerns to physicians, responsible parties, and appropriate clinical staff
• Order and manage medications through the pharmacy
• Supervise and provide direction to CNA staff
• Provide wound care and treatments as ordered
• Collect and process laboratory specimens
• Complete required assessments and documentation accurately and timely
• Communicate effectively with residents, families, physicians, pharmacy staff, and members of the interdisciplinary care team
• Follow all applicable nursing standards, facility policies, and state and federal regulations
• Provide compassionate, resident-centered care at all times
