Job summary

Sycamore Heights Health and Rehabilitation in Louisville, KY, is seeking a dedicated LPN to join our caring team across day and night shifts.

You will provide bedside care, monitor vital signs, administer medications, assist with personal hygiene, and document all patient information while collaborating with RNs and physicians. We value compassion, reliability, and strong communication.

Qualifications

• Current LPN license in the state.
• At least one year of LPN experience in a long-term care setting is preferred.

Responsibilities

• Provide bedside care to patients, including personal hygiene maintenance.
• Record the patients’ vital signs (blood pressure, pulse, respiratory rate).
• Give oral medications and injections to patients.
• Relate information between patients, RNs and physicians and document care.
• Maintain confidentiality and proper charting of medical records.

LPN Description:

• Provide bedside care to patients, including personal hygiene maintenance

• Record the patients’ vital signs (including blood pressure, pulse, and respiratory rate)

• Collect patient’s health information

• Relay information between patients, registered nurses, and physicians

• Give oral medications and injections to patients

• Test a patient’s blood sugar levels

• Urinary catheter insertion and care

• Wound care and dressing change

• Assist patients with personal hygiene

• Monitor for changes in patients

• Proper charting, documentation, and maintenance of patients’ medical records

• Asks questions when in doubt for clarification by the Registered Nurse.

• Acts as a role model for ancillary personnel.

• Maintains the confidentiality of the patient’s clinical records.

• Exceptional staff training and customer service skills

• Compassionate and friendly demeanor

LPN Requirements:

• Current LPN license in the state.

• At least one year of LPN experience in a long-term care setting is preferred.
