Position Details

Title: Regional Licensed Practical Nurse / Vocational Nurse / Nurse Case Manager (LPN)

Schedule: Daylight flex time, Monday-Friday, 8-hour days, no on-call rotation, flexible start times, holidays and weekends off.

Pay: Starting at $30/hour, commensurate with experience, with time-and-a-half after 40 hours/week.

Locations: Philadelphia, PA / Jenkintown, PA

Position for Residential Group Homes. Not a bedside nursing role; offers flexibility and a supportive environment.

Responsibilities

• Coordinate client discharges, conduct intake assessments, audit medication records, manage new medication orders, and train staff on diagnoses or procedures.
• Understand basic nursing concepts, including pharmacology, and manage multiple tasks efficiently.
• Monitor health conditions, administer medications, document care, and oversee staff medication administration.
• Provide medical treatments, conduct assessments, and respond to changes in health status.
• Train staff on infection control, medication, and safety protocols.
• Act as liaison with medical providers and advocate for individuals served.
• Train individuals on self-medication, treatments, STD prevention, and smoking cessation.

Qualifications

• Associate’s Degree in Nursing or related field, or graduate of an accredited LPN program.
• One year of nursing experience with the target population.
• Current state LPN or LVN license.
• CPR/FA certification (or equivalent).
• Effective communication and organizational skills.
• Computer literacy (email, Excel, Word); knowledge of 6400 regulations is a plus but training provided.
