Remote - Licensed Practical Nurse - LPN - LVN
CareHarmony's Care Coordinators (LPN) (NLC) (LVN) work comprehensively with providers to deliver value-based care management initiatives for their patients.
We are seeking an experienced Licensed Practical Nurse - LPN (LPN) (NLC) (LVN) with at least 3 years of direct patient-facing experience. You will have experience identifying resources and coordinating needs for chronic care management patients.

Responsibilities
- Manage patient census with a resolution-driven approach to close gaps in clinical and non-clinical care.
- Identify and coordinate community resources beneficial to patient care.
- Provide patient education and improve health literacy on chronic condition management.
- Perform medication management, including identifying concerns, reconciliation, adherence, and coordinating refills.
- Ensure timely delivery of services such as Home Health, DME, Home Infusion, and other critical needs.
- Resolve patient questions and foster open dialogue to understand needs.
- Assist with referrals and appointment scheduling.
Additional Requirements
- Active Compact/Multi-State LPN license.
- Technical aptitude, including proficiency with Microsoft Office Suite.
- Excellent written and verbal communication skills.
Preferred Qualifications
- Experience with Epic software.
- Bilingual abilities.
- Additional single-state licenses.
Remote Work Requirements
- Must have active high-speed Wi-Fi.
- Must have a home office or HIPAA-compliant workspace.
Physical Requirements
- This position is sedentary, requiring long periods of sitting.
- Ability to speak clearly and listen attentively, often by telephone.
- Ability to understand, process, and take thorough notes during calls.
