Job summary

Sage Rehabilitation Hospital of Ascension in Gonzales, LA is seeking a Licensed Practical Nurse (LPN) to join our compassionate care team. The role involves providing nursing support under the supervision of the RN and delivering high‑quality patient care in alignment with the interdisciplinary plan.

Qualifications

• Must be a graduate of an accredited School of Nursing.
• Current and active Louisiana Practical Nurse (LPN) license.
• Must be able to work a flexible schedule including 12-hour shifts.

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN.
• Communicates with the attending physicians, other team members to coordinate optimal care for the patient/family.
• Document all services provided and submit them on appropriate forms and report changes in patient condition and need to RN.
