Job summary

Sage Rehabilitation Hospital of Ascension in Gonzales, LA is seeking a Licensed Practical Nurse (LPN) to join our patient‑focused team.

Under RN direction, you will provide nursing care, coordinate with physicians and the care team, document services, and monitor patient condition. A Louisiana LPN license and graduation from an accredited School of Nursing are required, with a flexible schedule including 12‑hour shifts.

Qualifications

• Must be a graduate of an accredited School of Nursing.
• Current and active Louisiana Practical Nurse (LPN) license.
• Must be able to work a flexible schedule including 12 hour shifts.
• Have dependable transportation.

Responsibilities

• Provides nursing and supportive care in accordance with the Interdisciplinary Plan of Care under the direction of the RN.
• Communicates with the attending physicians and other team members to coordinate optimal care for the patient/family.
• Document all services provided and submit them on appropriate forms.
• Report changes in patient condition and need to RN.
• Great time management skills.
