Job summary

Mountain City Rehab Center in Frostburg, MD is seeking a licensed practical nurse to provide high-quality direct care to residents in a 99-bed skilled nursing facility. You will work under the Nurse Manager and DON, collaborating with an interdisciplinary team to ensure optimal resident outcomes.

Responsibilities include administering medications, creating and evaluating care plans, documenting progress, and supervising GNAs.

Qualifications

• Current Maryland LPN license or Multistate license.
• CPR/BLS certification (or willing to obtain on hire).
• Experience in long-term care or skilled nursing is preferred but not required.

Responsibilities

• Develop, implement, and evaluate individualized care plans.
• Document resident status and responses per policy and regulations.
• Monitor changes in resident condition and inform RN/physician as needed.

Job Summary

We are seeking a positive, caring Licensed Practical Nurse to join our team at the Mountain City Rehab Center, a beautiful 99-bed skilled nursing facility in Frostburg, Maryland. As an LPN, you will provide high-quality direct care to our residents, under the supervision of our Nurse Manager and DON. You will work collaboratively with our interdisciplinary team to ensure optimal resident outcomes.

Responsibilities

• Provide direct nursing care to residents, including administering medications, treatments, and monitoring vital signs
• Assist with the development, implementation, and evaluation of individualized care plans
• Document resident status, treatments, and responses in accordance with facility policy and state/federal regulations
• Monitor residents for changes in condition and report promptly to the RN or physician
• Collaborate with interdisciplinary team members, residents, and families to ensure optimal resident outcomes
• Supervise and support Geriatric Nursing Assistants (GNAs) in the delivery of care
• Maintain a clean, safe, and therapeutic environment for residents
• Ensure compliance with infection control, safety, and HIPAA policies
• Participate in quality assurance activities and staff meetings
• Fulfills the role of charge nurse when assigned

Requirements

• Current and valid LPN license in the state of Maryland or Multistate
• Graduation from an accredited Practical Nursing program
• CPR/BLS certification (or willing to obtain upon hire)
• Prior experience in a long-term care or skilled nursing facility preferred but not required
