Job summary

Frostburg Rehab Center in Frostburg, MD is seeking a Licensed Practical Nurse (LPN) to deliver compassionate, hands-on nursing care under the supervision of an RN or physician. You will monitor residents, administer medications, provide wound care, and help develop and update individualized care plans with the interdisciplinary team.

Strong clinical skills, attention to detail, and excellent communication are essential.

Qualifications

• Active LPN license in Maryland or multi-state.
• Experience in long-term care or skilled nursing preferred - new grads welcome!

Responsibilities

• Deliver safe, timely, and effective nursing care to residents under the direction of an RN or physician.
• Monitor patients' health, administer medications, and provide wound care and other treatments.
• Collaborate with interdisciplinary teams to develop and update individualized care plans.
• Support residents and their families with education, comfort, and reassurance.
• Maintain accurate documentation and comply with regulatory standards.
• Follows the plan of care as developed by the RN staff nurse, MDS Coordinator(s), or RN members of the Nursing leadership team.
• Observes safety policies and reports any safety concerns.
• Demonstrates respect of resident rights and dignity.
• Fulfills the role of charge nurse when assigned.
• Supervise and support GNAs in the delivery of care.
• Maintain a clean, safe, and therapeutic environment for residents.
• Ensure compliance with infection control, safety, and HIPAA policies.
• Participate in quality assurance activities and staff meetings.
