Ideal candidates hold an active Maryland LPN license, complete PN program, CPR/BLS, and have strong communication.

Qualifications

• Must have active Maryland LPN license or Multistate.
• Graduation from an accredited Practical Nursing program.

Responsibilities

• Provide direct nursing care to residents, including administering medications and monitoring vital signs.
• Assist with development, implementation, and evaluation of individualized care plans.
• Document resident status, treatments, and responses per policy and regulations.
• Monitor residents for changes and report to RN or physician.
• Collaborate with interdisciplinary team to ensure optimal outcomes.
• Supervise and support GNAs in delivering care.
• Maintain a clean, safe, and therapeutic environment.
• Ensure compliance with infection control, safety, and HIPAA policies.
• Participate in quality assurance activities and staff meetings.
• Fulfill the role of charge nurse when assigned.

Job Summary: We are seeking a positive, caring Licensed Practical Nurse to join our team at the Mountain City Rehab Center, a beautiful 99-bed skilled nursing facility in Frostburg, Maryland. As an LPN, you will provide high-quality direct care to our residents, under the supervision of our Nurse Manager and DON. You will work collaboratively with our interdisciplinary team to ensure optimal resident outcomes.

Responsibilities

• Provide direct nursing care to residents, including administering medications, treatments, and monitoring vital signs
• Assist with the development, implementation, and evaluation of individualized care plans
• Document resident status, treatments, and responses in accordance with facility policy and state/federal regulations
• Monitor residents for changes in condition and report promptly to the RN or physician
• Collaborate with interdisciplinary team members, residents, and families to ensure optimal resident outcomes
• Supervise and support Geriatric Nursing Assistants (GNAs) in the delivery of care
• Maintain a clean, safe, and therapeutic environment for residents
• Ensure compliance with infection control, safety, and HIPAA policies
• Participate in quality assurance activities and staff meetings
• Fulfills the role of charge nurse when assigned

Requirements

• Current and valid LPN license in the state of Maryland or Multistate
• Graduation from an accredited Practical Nursing program
• CPR/BLS certification (or willing to obtain upon hire)
• Prior experience in a long-term care or skilled nursing facility preferred but not required
• Strong clinical skills and attention to detail
• Excellent communication and interpersonal skills
• Ability to work effectively in a team environment
