Job summary

South Mountain Rehab Center in Boonsboro, Maryland is seeking a Licensed Practical Nurse (LPN) to join our Skilled Nursing team. You will supervise staff and safeguard residents’ health, safety, and welfare with a commitment to high-quality care.

The role emphasizes collaboration with RNs and other professionals, medication administration, wound care, and maintaining accurate documentation. We welcome new grads with a strong service orientation and excellent communication skills.

Qualifications

• Active LPN license in Maryland or Multistate.
• Experience in long-term care or skilled nursing preferred – new grads welcome!
• Strong clinical skills, attention to detail, and a heart for service.
• Ability to thrive in a fast-paced, team-oriented environment.
• Excellent communication and interpersonal skills.

Responsibilities

• Deliver safe, timely, and effective nursing care to residents under the direction of an RN or physician.
• Monitor patients' health, administer medications, and provide wound care and other treatments.
• Collaborate with interdisciplinary teams to develop and update individualized care plans.
• Support residents and their families with education, comfort, and reassurance.
• Maintain accurate documentation and comply with regulatory standards.
• Follows the plan of care as developed by the RN staff nurse, MDS Coordinator(s), or RN members of the Nursing leadership team.
• Observe safety policies and report any safety concerns.
• Demonstrate respect of resident rights and dignity.
• Fulfill the role of charge nurse when assigned.
• Supervise and support CNAs in the delivery of care.
• Maintain a clean, safe, and therapeutic environment for residents.
• Ensure compliance with infection control, safety, and HIPAA policies.
• Participate in quality assurance activities and staff meetings.
