Job summary

Humantouchhealth is seeking compassionate Licensed Practical Nurses (LPNs) for PRN home health opportunities throughout Fredericksburg and surrounding communities.

Provide skilled nursing visits in patients' homes, perform focused assessments, administer medications, and monitor healing. Join a team with flexible scheduling, area selection, mileage reimbursement, and consistent patient opportunities.

Qualifications

• Active Virginia LPN license
• Graduate of an accredited nursing program
• Current CPR/BLS Certification
• Valid Driver's License and reliable transportation
• Strong nursing, communication, and documentation skills
• Ability to work independently in patients' homes
• Home Health Experience preferred, but not required
• WellSky Experience is a Plus

Responsibilities

• Provide skilled nursing visits in patients' homes
• Perform focused nursing assessments
• Administer medications and treatments as ordered
• Provide wound care and monitor healing
• Monitor vital signs and changes in patient condition
• Reinforce medication and disease-management education
• Educate patients and caregivers
• Communicate changes or concerns to the supervising RN and clinical team
• Coordinate care with other members of the interdisciplinary team
• Complete accurate and timely documentation using WellSky

You do not have to cover the entire territory. Choose the areas that work best for you and build your caseload around your availability.
