Job summary

Trinity Health Corp. is seeking a Home Health Licensed Practical Nurse (LPN) to deliver skilled nursing care to clients in the home setting in Fort Lauderdale, FL, with supervision by a registered nurse.

The role emphasizes autonomy, meaningful 1:1 patient relationships, and a path for professional growth. You will be responsible for delivering care per the physician’s plan, with opportunities to advance clinically or into leadership roles within a nurse-led team.

Qualifications

• Minimum of one year experience in an acute care setting; home healthcare preferred.
• Graduate from an accredited or state approved practical nursing program.
• Current licensure in the State of Maryland.
• CPR certification required.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
• Must have current Driver’s license and reliable transportation to and from work site.

Responsibilities

• Deliver skilled nursing care to clients in accordance with the plan of care prescribed by the physician and assigned by the supervising RN.
• Provide top-of-license care with greater flexibility, autonomy, and meaningful 1:1 patient relationships.
• Engage in 1:1 patient care and build real relationships with patients and families.

Minimum Qualifications
