Job summary

Holy Cross Home Care and Hospice (Maryland) is seeking a Home Health LPN to deliver skilled nursing care in the patient’s home, following the physician’s plan of care under the supervision of an RN. You will provide care with autonomy and build meaningful 1:1 patient relationships while navigating a fast-paced home health setting.

Qualifications include an active Maryland LPN license, CPR, and at least one year of acute care experience; home health experience is preferred.

Qualifications

• Minimum one year of acute care experience.
• Home healthcare experience preferred.
• Graduate from an accredited or state-approved school of practical nursing.
• Current Maryland LPN license.
• CPR certification required.
• NAPNES or equivalent medication exam before administering medications.
• Valid driver’s license and reliable transportation to and from work site.

Responsibilities

• Deliver skilled nursing care per plan of care.
• Provide meaningful 1:1 patient care with autonomy.

Minimum Qualifications

• Must have a minimum of one (1) year experience in an acute care setting.
• Home healthcare experience preferred.
• Graduate from an accredited or state approved school of practical nursing
• Current licensure in the State of Maryland
• CPR certification required.
• Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
• Must have current Driver’s license and reliable transportation to and from work site.
