Job summary

Holy Cross Home Care and Hospice (Maryland) is seeking a Home Health LPN to deliver skilled nursing care in patients’ homes. You’ll follow the physician’s plan of care and work under the supervision of a registered nurse, enjoying autonomy and meaningful 1:1 patient interaction.

Candidates should have at least one year of acute care experience, Maryland LPN licensure, CPR certification, and a valid driver's license with reliable transportation.

Qualifications

• Minimum of one year experience in an acute care setting.
• Home healthcare experience preferred.
• Graduate from an accredited or state approved school of practical nursing.
• Current licensure in the State of Maryland.
• CPR certification required.
• Must have passed NAPNES or equivalent medication exam before administering medications.
• Must have a valid driver’s license and reliable transportation to work site.

Responsibilities

• Deliver skilled nursing care to clients per the plan of care prescribed by the physician.
• Provide care at the top of your license with autonomy and 1:1 patient relationship.
• Utilize Epic EHR system for efficient documentation and care coordination.
• Support and collaborate with supervising RN to ensure quality outcomes.

Minimum Qualifications

• Must have a minimum of one (1) year experience in an acute care setting.
• Home healthcare experience preferred.
