Job summary

Fairfield Memorial Hospital Association in Fairfield, IL is seeking a licensed practical nurse (LPN) for as-needed (PRN) shifts. You will perform direct patient care, administer medications, assess residents, document in the EHR, and collaborate with a multidisciplinary team to ensure high-quality care.

Qualifications

• Current LPN license in Illinois is required.
• BLS certification is required.

Responsibilities

• Provide direct nursing care to residents per physician orders and care plans.
• Perform ongoing resident assessments and document in the EHR.
• Administer medications and treatments per nursing standards.
• Monitor changes in resident condition and communicate with physicians and RNs.
• Update individualized care plans with the care team.
• Educate residents and families about care and hospital guidelines.
• Supervise CNAs and support staff to ensure quality care.
• Perform IV therapy within LPN scope and facility policy.
• Ensure shift-to-shift communication about status and orders.
• Maintain infection prevention protocols and regulatory compliance.
• Advocate for residents and uphold their rights and dignity.

KEY RESPONSIBILITIES

• Provide direct nursing care to residents in accordance with physician orders, nursing standards, and established care plans.
• Perform ongoing resident assessments and document findings in the electronic health record (EHR).
• Administer medications and treatments safely and accurately according to established nursing standards.
• Monitor and communicate changes in resident condition to physicians and Registered Nurses.
• Implement and update individualized resident care plans in collaboration with the interdisciplinary care team.
• Provide resident and family education related to care, treatment plans, and facility guidelines.
• Supervise and direct Certified Nursing Assistants and support staff to ensure quality resident care.
• Perform IV therapy duties within the Illinois LPN scope of practice and facility policies.
• Ensure thorough shift to shift communication regarding resident status, physician orders, and pending care needs.
• Maintain compliance with infection prevention protocols, regulatory requirements, and hospital policies.
• Serve as a resident advocate while promoting resident rights, dignity, and well-being.
• Participate in department quality improvement initiatives and ongoing professional development activities.

COMPENSATION:

$34.00 per hour + Shift Differential (Weekday Night: $2.00 per hour; Weekend: $3.00 per hour; Weekend Night: $5.00 per hour)

QualificationsEDUCATION/QUALIFICATIONS

• Required: Current Licensed Practical Nurse (LPN) licensure in the State of Illinois; Current Basic Life Support (BLS) certification.
• Preferred: Long Term Care, Skilled Nursing, or Geriatric Care experience; Experience with electronic health records in a skilled nursing setting.

EXPERIENCE

• Preferred: Previous nursing experience in a long-term care, skilled nursing, rehabilitation, or acute care setting; Experience supervising CNAs or other support personnel.
