Job summary

Nashville Center For Rehabilitation and Healing in Berry Hill, TN seeks a Licensed Practical Nurse (LPN) for full-time rotating 12-hour shifts (7-7). You will administer medications, monitor vitals, and provide resident care while documenting outcomes and educating residents and families.

Supervise CNAs, maintain clear communication with the care team, and contribute to a high-quality LTC environment. Eligible applicants hold a TN LPN license and are ready for immediate employment.

Qualifications

• Current Tennessee LPN license is required.
• Experience with skilled nursing or long-term care preferred.
• Ability to administer medications including IVs and monitor outcomes.

Responsibilities

• Provide care for residents in Skilled Unit and Long Term Care Unit.
• Administer medications, including IVs, and record vitals.
• Monitor resident condition and document outcomes.
• Educate residents and families about care plans.
• Communicate with physicians, NPs, families and care team.
• Supervise CNAs during shifts.
