Description:
Total # of Facility Beds: 98
Unit Name and Cost Center: LONG TERM CARE (OTTER CREEK EAST & WEST)
Unit Specialty: LONG-TERM CARE

Assignment for 16-32 weeks
LPN for Helen Porter Rehab & Nursing
8H Days, will be 72H biweekly: 4 x 8s one week and 5 x 8s the next. Every other weekend, floating required. Cat on unit - cannot be allergic.

Questionnaire attached to position files needs to be reviewed with interested candidates and uploaded in system for IVS review.
Requirements:
Dementia capable care certification preferred
A minimum of 1 year of experience
Prior travel required
EPIC experience from within the past 2 years is required and nonnegotiable

No block schedules for any reason
This facility requires ALL work history information (bed count, EMR, facility type, etc.) to be complete in order to submit
