We are seeking an LPN for phone triage & intake. This is a remote, part-time position working weekends, with shifts of 10 or 12 hours. Must be able to work both Saturdays & Sundays.

Responsibilities:

The LPN will provide care for patients in various settings, including private homes, Skilled Nursing Facilities, and Assisted Living Facilities. The role involves psychosocial assessments, interventions, and connecting patients with community resources. The LPN collaborates with an interdisciplinary team, offering expertise and support.

Qualifications:

• Current LPN licensure in good standing in applicable states, with at least one year of experience
• Ability to work collaboratively and independently
• Strong communication skills via phone, email, and meetings
• Knowledge of state and federal healthcare regulations
• Effective time management skills
• Willingness to drive; valid driver’s license and auto insurance required
