SCOPEOFROLE: Diagnosisandmanagementofacute,chronicandcomplexhealthproblems;assessing,diagnosingandtreatingillnesses,educatingtheshelterclientsandstaffondiseaseprevention.Providehealtheducationandcounselingtoindividualsresidingintheshelter.ESSENTIALDUTIES&RESPONSIBILITIES: MeetswithProgramDirector,DirectorofSocialServices,Psychiatrist,ProgramNurses,andSocialServicestafftocoordinateshelterresidentstoreceiveongoingtreatmentinthecommunity. SuperviseMedicationCoordinatorandassistPsychiatristandPsychiatricNursePractitionerasneeded. Administermedicationsanddocumentadministrationasrequired. Takevitalsigns,suchasbloodpressure,temperatureandweight Assistwithdiabetesmedicationandtreatment Takemedicalhistoriesandenterinformationincomputersystems CoordinatewithCaseManager,SeniorCaseManagerandDirectorofSocialServicestoensurepropercare,nutritionandtreatmentplansarefollowed. Followupwithcommunityhealthcareproviderstoensuremedicaltreatmentplansaredocumented,monitoredandfollowed MonitorMARsandmedicationstorage. WorkwithresidentsandCaseManagersinfillingprescriptionsandensuringrefillsareorderedasnecessary. Ensureconsumersafety,assessconsumerreactionstomedications,assistwithconsumernutritionalneeds. Ensurephysicianordersarecurrent,properlydocumentedandcommunicatedtostaff Monitorclientmedicalequipmentforsafetyandproperfunctioning. Attendandparticipateinregularstafftrainingandschedulingmeetings. Maintainconfidentialityandprivacyregardingallclients’information. Completedailydocumentationofallagencyrequiredmaterial. Completeincidentreportsinaccordancewithpolicyandprocedures. Understandandimplementtheagency’sphilosophy,policiesandmission. Understandandfollowtheagencychainofcommunication. Ensurethesafetyandwell‐beingoftheclientsonadailybasis. Workcooperativelywithotherteammemberstocarryouttheprogramgoalsandobjectivesforeachclient. Reportanyactualorsuspectedabuse/neglectinaccordancewithpolicyandprocedure.
Prepareclientsforself‐medicationstatusandmonitorasneeded,healthassessmentreferrals. ComplywithallSUSEmployeePolicies,Procedures,andProtocols,SUSCorporateCompliancePolicies,andNYSDept.ofSocialServicesRules,Regulations,andStatutes. MaintainCPR/FirstAidCertification. Assistinbuildingevacuationduringfiredrillsandbuildingemergencies. Referresidentstoemergencycareifneeded,workwithemergencyresponders/emergencyroomstafftocoordinateemergencytreatment,coordinatetreatmentanddischargeplanningwithhospitalstaff. Aschedulethatincludeseveningsandweekendswillberequired.
