Responsibilities

The Licensed Practical Nurse (LPN) in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home.

• Provide services within the scope of practice as defined by state laws, in accordance with the plan of care, and in coordination with the healthcare team. May not alter the plan of care.
• Complete progress notes for each visit and submit them daily to the agency.
• Observe and report the patient’s nursing needs continuously. Instruct the patient’s family in care and maintaining a healthy environment.
• Observe, record, and report to the supervising nurse or physician the patient's response to treatment and any changes in condition.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice.
• Valid Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR Certification required.
