Responsibilities

The Licensed Practical Nurse (LPN) in Home Health provides quality patient care and performs technical skilled care in patients' homes.

• Provide services within the scope of practice defined by state laws, following the plan of care, and coordinating with the healthcare team. Do not alter the plan of care.
• Complete and submit progress notes after each visit.
• Observe and report the patient's needs and condition; instruct family members on patient care and maintaining a healthy environment.
• Record and report the patient's response to treatment and any changes in condition to the supervising nurse or physician.

Education and Experience

License Requirements:

• Current LPN license in the state of practice
• Valid driver's license, vehicle insurance, and access to a reliable vehicle or transportation
• CPR certification
