is seeking a compassionate and dedicated Week Night M-F On Call Licensed Practical Nurse (LPN) to join our team at our Central Pennsylvania Senior Living office. This office services geriatric clients on a per visit basis in senior living communities throughout Dauphin County. One year of prior clinical experience as an LPN is required.

As a home care nurse, you will be an integral part of a multi-disciplinary healthcare team providing skilled nursing and rehabilitative care, enabling clients to remain at home.

Licensed Practical Nurse (LPN) Responsibilities:

1. Conduct home visits to clients in designated territories.
2. Administer medication, perform wound care, treatments, and procedures.
3. Monitor client conditions and report changes to management.
4. Follow doctors' orders accurately and document appropriately.
5. Perform client assessments as needed.
6. Manage cases and coordinate care.
7. Document observations, interventions, and evaluations using electronic medical records on a tablet.

Qualifications:

• Current Pennsylvania LPN license.
• Minimum one year of recent, verifiable LPN experience.
• Graduation from an accredited nursing program.
• Home care experience preferred but not required.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Proficient computer skills; EMR experience preferred.
• Ability to travel to assigned cases.
