Job summary

People, Incorporated in Fall River, MA is seeking a Licensed Practical Nurse to work under the supervision of the Registered Nurse and provide health care services in Adult Day Services.

Duties include administering medications, coordinating care with health professionals, monitoring health status, and documenting in records, with opportunities to train staff and participate in required trainings.

Qualifications

• Attainment of a Massachusetts LPN license.
• Ability to maintain current licenses, CPR, first aid, and health records on file.
• Knowledge and skill from at least two years in human services.
• Ability to communicate clearly and work both independently and in a team.

Responsibilities

• Administer medications and treatments prescribed by the PCP during program participation.
• Coordinate each individual's care with other health professionals and residence.
• Monitor health status and document findings in the individual's record.
• Document medical conditions or findings every six months as delegated.
• Report changes in condition to the RN or Program Director and PCP when needed.
• Oversee implementation of PCP recommendations and interdisciplinary therapies.
• Provide training to staff on individuals' medical protocols.
• Participate in required staff meetings and trainings and maintain all required trainings.
