Sevita’s MENTOR Adult Day Health program in Marlborough, MA, is hiring a Licensed Practical Nurse (LPN) for the Aging Well Adult Day Center. You’ll provide compassionate, person-centered care to adults and seniors while enjoying a predictable daytime schedule.

• Graduate of an accredited Licensed Practical Nurse (LPN) program.
• Current Massachusetts LPN license required.
• Current CPR and First Aid certification.
• Strong communication, organizational, and interpersonal skills.

Responsibilities

• Assist with health assessments and participant wellness monitoring.
• Support development and implementation of individualized care plans.
• Administer medications and treatments per physician orders and protocols.
• Promote health education, wellness initiatives, and preventative care practices.
• Collaborate with Registered Nurses, physicians, specialists, families, and team members for coordinated care.
• Maintain a safe, healthy, person-centered environment for all participants.

MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs.
