Qualifications

• Massachusetts LPN license required.
• High school diploma or GED.
• Nursing degree from a community college or technical school.
• Valid driver’s license.
• One year of psychiatric or special needs experience preferred.

Responsibilities

• Monitor residents’ daily health needs and provide nursing support.
• Coordinate preventive, specialized, and emergency medical appointments.
• Coordinate and monitor medication ordering, administration, and documentation.
• Maintain accurate medical records and assist with required program reports.
• Participate in intake, discharge, and hospital discharge planning.
• Collaborate with clinical staff, families, and community providers.
