Job summary

Responsibilities include patient assessment, care plan modification with the team, and upholding safety standards. Requires Florida LPN license and BLS; travel to centers may be needed.

Qualifications

• LPN Diploma and transcripts are required.
• Valid Florida LPN license is required.
• BLS certification is required.

Responsibilities

• Record vital signs following standard protocol.
• Administer medications and vaccines; wound care as needed.
• Monitor patient conditions and report changes promptly.
• Collaborate with the care team to assess needs and modify care plans.
• Promote patient safety by following protocols and standards of care.
• Manage telephone encounters, triage, referrals, and care coordination.
• Support providers by managing patient flow and health education.
• Administer injections and IV fluids as certified; document per protocol.
• Supervise and assist ancillary staff as needed.
• Log, store, and handle medications and vaccines.
• Maintain required training (CPR, Infection Control, OSHA, Relias).
• Troubleshoot and report significant changes to supervisor.
• Maintain recall programs for immunizations and screenings.
