Job summary

Community Health Centers is seeking a dedicated Licensed Practical Nurse (LPN) to support our clinical teams in Florida. The role involves preparing patients for physician interaction, coordinating the care team, and assisting with lab work, immunizations, and routine duties.

You will help manage patient flow, document communications, and support quality patient care across flexible hours, including some evenings and weekends. A validFlorida LPN license and BLS are required.

Qualifications

• Must have LPN license in Florida.
• Minimum 2 years’ experience as an LPN preferred.
• Current/valid BLS certification, or obtained within 30 days of employment.
• Bilingual skills preferred.

Responsibilities

• Coordinate and facilitate structured communication to care team members and providers.
• Participate in team meetings to address patient care and operational requirements.
• Lead huddles emphasizing a team-based approach to care.
• Manage patient care activities and schedules within the care team.
• Administer immunizations and medications as ordered with safe medication practices.
• Assist clinicians with exams and procedures and provide patient education.
