Elara Caring is seeking a Licensed Vocational Nurse (LVN) for PRN coverage in the Ware/Palmer, MA area. The role focuses on delivering high-quality, compassionate home health care, following care plans, and coordinating with the RN Case Manager to adapt to patient needs.

Candidates must be licensed LVNs, have at least 1 year of clinical care experience, and be able to travel between patient homes. A valid driver’s license or reliable transportation is required.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of clinical care experience.
• Ability to frequently lift up to 50 pounds, push/pull, and move patients/equipment.
• Must possess a valid driver's license or reliable transportation for travel between patient homes.

Responsabilités

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Coordinate with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.
