Job summary

Elara Caring is seeking a licensed LVN to join our home health team in the Ware/Palmer area on a PRN basis. You will follow individualized care plans, coordinate with RNs and physicians, and provide compassionate, high-quality care to patients in their homes.

The ideal candidate holds an LVN license, has 1 year of clinical experience, and can lift up to 50 pounds while transporting equipment. A valid driver's license and reliable transportation are required.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing.
• 1 year of experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, or have reliable transportation to travel between patient homes.
• Experience in a hospice or home health environment is preferred.

Responsibilities

• Follow each patient's individualized care plan and physician orders.
• Monitor patient progress and report changes or safety concerns to the care team.
• Help patients and caregivers learn self-care techniques to support their health.
• Work with the RN Case Manager to update care plans as patient needs change.
• Document medication regimens and update medication profiles according to agency guidelines.
• Communicate with physicians, patients, families, and the care team to ensure coordinated care.
• Help prevent avoidable hospitalizations and emergency room visits through proactive patient care.
• Deliver compassionate, high-quality care that supports positive patient outcomes.

• The compensation range is $30 Per Point, which can include additional earnings for work beyond quota, bonuses, on-call, or other performance-based compensation variables.
