Elara Caring is seeking a Licensed Vocational (LVN) Nurse for a PRN role in the Springfield, MA area. You will follow patient care plans, communicate with the care team, and support families with self-care techniques to improve outcomes.

Responsibilities include monitoring patient status, updating care plans with RN supervision, documenting medications, and coordinating with physicians. Prior hospice/home health experience is preferred and a valid LVN license is required.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, or Associate).
• 1 year of experience in a clinical care setting.
