Job summary

Amedisys is seeking a Licensed Practical Nurse to join our home health team in Southeast Sussex County, DE. This full-time role offers meaningful patient care, flexible scheduling and growth within a national leader in home health and hospice.

The ideal candidate has at least one year of LPN experience, current state license, CPR certification, and a valid driver's license with reliable transportation. You’ll verify plans of care, teach self-care, and document meticulously.

Qualifications

• At least one year of clinical LPN experience.
• Current LPN license for the state(s) assigned.
• Current CPR certification.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN to ensure patient needs are met; notify if condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform a medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. Foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
