Job summary

Amedisys Home Health is seeking a Licensed Practical Nurse to provide high-quality in-home patient care in Southeast Sussex County, Delaware. This full-time role offers competitive pay and a $5,000 bonus, reflecting the cost of labor across markets and eligibility for various benefits.

Responsibilities include verifying care plans with RNs, collaborating with physicians, teaching self-care, checking vitals, and performing procedures as ordered.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify them promptly if patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g., Foley catheter changes).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
