Job summary

Amedisys is seeking a licensed practical nurse to join our team serving patients in Kent/Providence County, RI. This full-time, daytime role offers flexible, purpose-driven work in a home-health setting, with the option to work in patients' homes across Warwick and surrounding areas.

Responsibilities include care planning verification with RNs, medication reconciliation, vital signs, patient and caregiver instruction, and documentation. CPR certification and a valid state license are required.

Qualifications

• One year of clinical LPN experience.
• Current LPN license, specific to the state(s) you are assigned to work.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.

Responsibilities

• Verify plan of care with the RN prior to each visit.
• Collaborate with physician and RN frequently to ensure patient needs are met; notify when patient condition changes.
• Teach patients and caregivers self-care techniques.
• Check vital signs.
• Review medications and perform medication reconciliation at each visit.
• Perform specialized procedures ordered by physician (e.g. foley catheter change).
• Prepare equipment and materials for treatments using aseptic technique.
• Complete documentation timely and accurately.
