Job summary

Community Care Family Medicine in New York is seeking a dedicated LPN for a full-time, 40-hour workweek to join our clinical team. The successful candidate will administer vaccines, collect specimens, document conversations and patient data, and help maintain smooth patient flow.

Responsibilities include order entry, EMR documentation, assisting with procedures (PFT, EKG, nebulizer), and coordinating patient care under provider direction.

Qualifications

• Active NYS LPN license and valid credentials.
• Experience in a medical practice is preferred.
• Proficient with EMR systems and charting.
• Knowledge of infection control and patient confidentiality.

Job description

Our Community Care Family Medicine office is looking for a Full Time LPN to join their team! This is a 40 hour per week position.

Responsibilities

• Administer and record vaccines.
• Document verbal conversations with patients when appropriate.
• Document information into patient record in an accurate, legible and timely manner.
• Maintain timely patient flow.
• Prepare patient for visit as appropriate, based on office/provider requirements.
• Obtain specimens (i.e., urine, blood, etc.) from patients as directed by the provider.
• Accurate and timely order entry into the lab system.
• Perform clinical office procedures as directed by the provider (i.e. PFT, EKG, nebulizer, medication administration and immunizations)
• Process patient prescriptions.
• Discuss test results with patients per provider's instructions.

Compensation

$26.00 - $33.00 per hour

CCP salary ranges are designed to be competitive with room for professional and financial growth. Individual compensation is based on several factors unique to each candidate, such as work experience, qualifications, and skills. Some roles may also be eligible for overtime pay.

• Previous LPN experience in a medical practice or prior experience as a Medical Assistant or Patient Care Tech.
• Strong computer skills with experience using EMR.
